# United Concordia FEDVIP
> Federal dental insurance for eligible federal employees, federal annuitants, and military 
retirees — offering two comprehensive FEDVIP plans with no deductibles, no waiting 
periods, and free in-network preventive care through a national network of 278,000+ 
dentist locations.
United Concordia FEDVIP is administered by United Concordia Companies, Inc. under 
contract with the U.S. Office of Personnel Management (OPM) as part of the Federal 
Employees Dental and Vision Insurance Program (FEDVIP). With more than 50 years of 
dental benefits experience, United Concordia serves over 10 million members nationwide, 
including over 820,000 FEDVIP members, and has proudly served more than 3 million 
military members and their families. FEDVIP is an enrollee pay-all supplemental dental 
program separate from the Federal Employees Health Benefits (FEHB) Program. Enrollment 
is managed through BENEFEDS.gov. Coverage is effective the first day of the first pay period 
following enrollment.
United Concordia offers two FEDVIP plan options: the High Option Plan and the Standard 
Option Plan. Both plans include no deductibles, no waiting periods, free in-network 
preventive care, the Smile for Health®–Wellness benefit, the Pregnancy Benefit, and a
$2,000 dental accident lifetime maximum. The High Option Plan adds an unlimited annual 
maximum, three routine cleanings per year, a $2,500 dental implant annual maximum, a 
$3,000 orthodontic lifetime maximum covering adults and children, and broader major
service coverage. The Standard Option Plan includes a $1,000 annual maximum and a 
$1,500 orthodontic lifetime maximum for children to age 19.
Members may visit any licensed dentist in the U.S., with lower costs when using in-network 
providers. In-network dentists accept negotiated fees as payment in full, file claims on 
behalf of members, and have been credentialed through United Concordia's quality 
assurance process. 94.4% of FEDVIP members have two or more in-network dentists within 
10 miles of home or work. The network includes 278,000+ rural and urban office locations 
nationwide. United Concordia has active Dental Network URAC Accreditation.
Members can manage benefits 24/7 through the MyDentalBenefits online portal and 
mobile app (available on iOS and Android). Customer service representatives are available 
toll-free at 1-877-394-8224, Monday–Friday, 8 a.m.–8 p.m. ET. United Concordia's 
customer service team holds a 96% member satisfaction rate.
