For what amounts can dentists bill employees under the United
Concordia plan?
United Concordia's Advantage Plus network dentists accept
our reimbursements, also known as maximum allowable charges (or MACs),
as payment in full for covered services. United Concordia's Advantage Plus
network dentists can charge employees for applicable
coinsurance amounts, but they cannot balance bill for the
difference between their standard charges and the MACs. Non-network
dentists can bill for the difference between their standard
charges and the MACs, which means employees could save more by visiting
a network dentist.
How many routine visits per year are covered under
the plan? Two every 12 months.
Are referrals needed to visit a specialist?
No.
How can employees find out about covered services?
First, check online under
The Right Plan, where they can find the Benefits
Summary of the FEDVIP plan. They can also refer to United Concordia's
FEDVIP brochure for explanations of coverages and services.
Encourage them to always consult with their dentist on
treatments, covered services and out-of-pocket costs. And
they can call United Concordia’s dedicated customer

Do employees need to take an ID card or claims form
to the dentist? United Concordia FEDVIP
members are not required to bring their ID cards to the
dentist. However, each member's ID card displays his/her
individual unique member identifier (UMI), which could be
useful to the dentist to submit claims and/or retrieve
eligibility information.
If a member visits an Advantage Plus network
dentist, it is not necessary for him/her to bring a claim form; United
Concordia's network dentists take care of all paperwork.
However, employees may need to submit their own claims
if they visit a non-network dentist. They can print a
claim from the Forms & Publications page,
and should bring it to the dentist's office for their appointments.
What is the course of action when an employee disagrees
with a bill? Tell him/her to first talk with
the dentist. If questions still remain, have him/her call
United Concordia’s customer service representatives

Can United Concordia FEDVIP members visit any dentist?
Yes, United Concordia FEDVIP members may visit any dentist.
However, inside the United States and its territories, they could
experience reduced out-of-pocket costs by visiting an
Advantage Plus network dentist. (Please note that
United Concordia does not have participating dentists outside
the United States and its territories.)
Can United Concordia FEDVIP members visit a dentist outside the United
States and its territories?
Yes. However, United Concordia does not have participating
dentists outside the United States and its territories, so
members may experience higher out-of-pocket costs when
visiting these dentists.
What if a FEDVIP member needs to visit a dentist in an
emergency?
FEDVIP members may visit any dentist. However, inside
the United States and its territories, members could experience
reduced out-of-pocket costs by visiting an Advantage Plus
network dentist.
How can employees find a dentist in the United Concordia
Advantage Plus network? The
easiest way to find a dentist is to use our online
directory. Employees can simply click on
Find a Dentist at
www.uccifedvip.com
and search by dentist name,
specialty, city, county or ZIP code. They can
also call United Concordia at 1-877-FYI-UCCI,
and our customer service representatives will assist them
with information regarding a dentist’s membership
status, specialty and office location.
Can dentists be nominated to be part of the Advantage Plus
network? Yes,
we gladly accept nominations for new Advantage Plus
network dentists. To nominate
a dentist, use the
Nominate Your Dentist form on this site.
Dentists can also obtain an application for
Advantage Plus network participation by
calling our toll-free customer service line or by
downloading the application.
Does a dental specialist have to be in the United
Concordia Advantage Plus network as well?
Employees can visit any specialist; however, their costs may be
greater if they visit a non-network specialist.

How secure is your data?
Information security and the privacy of our enrollees is a
high priority for United Concordia. Through our experience
as the dental insurer for the TRICARE Dental Program, we use
the same information technology and physical
site security practices required by DIACAP to protect and serve
all of our customers. This includes safeguarding information
in our state-of-the-art data center, designed to withstand
significant natural, virtual and man-made forces.
How do our employees contact United Concordia with questions?
If questions are about dental treatments, please encourage your
employees to first discuss them with their dentist. If
general questions or clarification of benefits cannot be found
online, please have them call our dedicated customer service

We have employees who do not speak English. Do you have
translators in your customer service department?
Yes, we have bilingual English/Spanish customer service
representatives. We also have translation services
for 140 other languages.
What does United Concordia have available to help educate
federal employees about the dental program?
In addition to our special FEDVIP website that details
the plan, premiums and network information, we have developed
a video for the 2011 enrollment season that provides an overview
of United Concordia, available plan options, and information on how to enroll.
We also have a series of articles that explain some of the more
complicated dental insurance terms/coverage provisions such as
the alternate benefits provision, coordination of benefits and
predetermination of costs. We also have frequently
asked questions for members and an extensive glossary
of dental terms. All of these materials can be used in
newsletters or letters to employees about the FEDVIP plan.
What does United Concordia offer to help promote dental
health? We have a full library of resources
to promote dental health, including brochures and articles on
everything from bad breath to heart disease. All
of these resources are readily available online.
