How can I request a new ID card?

Members can print one anytime-just sign in to My Dental Benefits.

In fact, My Dental Benefits is available 24 hours a day and lets members:

  • View benefits and eligibility information
  • Check on the status of a claim
  • Review treatment history
  • Print ID cards
  • Find a network dentist

You can also log in to our mobile app to locate your ID card or call customer service at 1-877-FYI-UCCI to request a new ID card.

How can I check the status of my claim?

Members can see their latest claims and read their EOBs online by signing in to My Dental Benefits. They can even read up on what's covered and find a dentist in the Federal Dental Program Network.

In fact, My Dental Benefits is available 24 hours a day and lets members:

  • Find a network dentist

Do I have to file a claim after I visit the dentsit?

If you visit a Federal Dental Program Network dentist, you do not need to take a claim form, as our network dentists have agreed to file claims for you. However, it is a good idea to bring a Claim Form if you are visiting an out-of-network dentist.

Where can I get a copy of a claim form?

These are forms your dentist will complete to submit a claim to United Concordia. You should only need one if you are visiting an out-of-network dentist; our network dentists will file claims for you.
