With more than 50 years of dental benefits experience, United Concordia serves over 10 million members nationwide, including over 820,000 FEDVIP members, and has proudly served more than 3 million military members, their families, veterans, and government employees. United Concordia holds active Dental Network URAC Accreditation and maintains a 96% member satisfaction rate. FEDVIP is an enrollee pay-all supplemental dental program. It is separate from the Federal Employees Health Benefits (FEHB) Program and the Postal Service Health Benefits (PSHB) Program. Enrollment and changes are managed through BENEFEDS.gov. --- ## What is FEDVIP? - [How to Enroll](https://www.uccifedvip.com/how-to-enroll) The Federal Employees Dental and Vision Insurance Program (FEDVIP) is available to eligible federal and postal employees, retirees, their eligible family members, and military retirees on an enrollee-pay-all basis. The program allows dental insurance to be purchased on a group basis, providing comprehensive coverage at competitive premiums. Employees must be eligible for FEHB or PSHB in order to enroll in FEDVIP — they do not need to actually be enrolled in FEHB/PSHB, only eligible. Annuitants are eligible to enroll in FEDVIP regardless of their FEHB/PSHB status. FEHB (Federal Employees Health Benefits) is the largest employer-sponsored group health insurance program in the world, covering federal employees, retirees, former employees, family members, and former spouses. PSHB (Postal Service Health Benefits) provides health benefits to eligible Postal Service employees, annuitants, and their eligible family members. --- ## Eligible Members - [How to Enroll](https://www.uccifedvip.com/how-to-enroll) **Federal Civilians:** Employees eligible for FEHB/PSHB are eligible for FEDVIP dental coverage. Annuitants are eligible. Premiums are paid pre-tax for employees. Dependent children are covered until age 22. **Uniformed Services:** Most retirees are eligible for dental coverage. Active duty uniformed service members are not eligible for dental coverage. Premiums are paid posttax. Coverage automatically renews year to year unless a change is made during Open Season or due to a qualifying life event (QLE). **How to enroll:** During Open Season or following a qualifying life event, go to BENEFEDS.gov and follow the instructions to select United Concordia Dental's High or Standard FEDVIP plan. If you do not have computer access, call 1-877-888-3337. **Coverage start date:** Coverage periods run January 1 through December 31. The coverage period begins on January 1, 2026. Coverage is effective the first day of the first pay period following enrollment submission. **No waiting periods:** There are no waiting periods to begin using coverage. **Missed Open Season:** If Open Season is missed, enrollment must wait until the next FEDVIP Open Season, unless a qualifying life event applies. **Qualifying Life Events (QLEs):** The following QLEs allow enrollment within 31 days before to 60 days after the event: marriage, loss of other dental/vision coverage, acquiring a family member, return from leave without pay, return from active duty (member or spouse), and restoration of annuity/compensation. Moving out of a regional plan's service area or loss of a family member allows plan changes. Returning to active duty allows cancellation of coverage. Provide your name, personal information, agency or retirement system, and chosen dental plan. **Premium payment:** Federal employees pay premiums through payroll deduction with pre-tax dollars. Annuitants pay via deductions from annuity payments. There is no government contribution — enrollees are responsible for 100% of the cost. Members with diabetes, heart disease, rheumatoid arthritis, lupus, oral cancer, or who have had a stroke or organ transplant are eligible for additional periodontal services, including one extra periodontal maintenance visit per year, scaling and root planing, and periodontal surgery — all covered at 100% in-network and out-of-network. Members must register through MyDentalBenefits at uccifedvip.com/smile-for-health-wellness. **Pregnancy Benefit:** Covered members who are pregnant receive one extra cleaning covered at 100%, plus coverage for additional procedures necessary to treat or manage gum disease during pregnancy, at no additional cost. Members must register through MyDentalBenefits at uccifedvip.com/pregnancybenefit. **Sonicare Discount:** United Concordia has partnered with Philips to offer FEDVIP members 20% savings on select Sonicare electric toothbrushes and air flossers for adults and kids. --- ## Understanding Benefits - [Our Plans](https://www.uccifedvip.com/our-plans) **Annual maximum:** The maximum dollar amount the plan will pay toward an individual's dental care during a plan year. The High Option plan has an unlimited annual maximum (exclusive of implant services and orthodontics) with a $2,500 annual maximum for dental implant services. The Standard Option plan has a $1,000 annual maximum. **Lifetime maximum:** The maximum benefit amount available for a specific service or group of services. The High Option plan has a $3,000 orthodontic lifetime maximum. The Standard Option plan has a $1,500 orthodontic lifetime maximum. Both plans have a $2,000 dental accident lifetime maximum. **Orthodontic continuity note:** If a United Concordia participant is enrolled as of December 31, 2025, is undergoing active orthodontic treatment, and renews enrollment in 2026, claims for that treatment will continue to be processed under the 2025 plan's lifetime orthodontia maximum and co-insurance for the duration of the approved treatment plan. **Explanation of Benefits (EOB):** A statement sent to members each time a dental claim is processed. The EOB shows expenses submitted by the dentist and how the claim was processed, including services covered, amounts paid by United Concordia, amounts not covered, and benefit limitations. The EOB includes four sections: claim information, service and coverage information, explanations of limitations or exclusions, and patient summary. Members can register in MyDentalBenefits to receive paperless EOBs. **Predetermination:** An estimate of costs (also called a "Pre-D") obtained before treatment begins. It shows what services will be covered, what the plan will pay, and what the member will owe. United Concordia recommends requesting a predetermination for all proposed treatments over $300. The dentist submits the treatment plan and both the dentist and member receive a predetermination notification. **Dental accident benefit:** A dental accident is an injury to sound natural teeth caused by a violent external force such as a fall or blow to the mouth. United Concordia pays 100% of the program allowance for covered services related to accidental dental injuries, up to a $2,000 lifetime maximum. This benefit is separate from the annual maximum. **Implant prosthetic:** Typically composed of a post attached to the jawbone and a crown placed on top. Implant-related procedure codes may be covered under Class C services if dentally necessary. The High Option plan includes a $2,500 annual implant maximum. **Plan changes mid-treatment:** Services initiated, prescribed, or performed prior to the effective coverage date are not covered. Federal employees pay premiums through payroll deduction using pre-tax dollars. Annuitants pay via annuity payment deduction. There is no government contribution toward FEDVIP premiums. --- ## Network - [Find a Dentist](https://www.uccifedvip.com/find-a-dentist/#/) United Concordia's FEDVIP network includes 278,000+ rural and urban office locations nationwide. 94.4% of FEDVIP members have two or more in-network dentists within 10 miles of home or work. All in-network dentists undergo thorough credentialing through United Concordia's quality assurance process. **Benefits of visiting an in-network dentist:** - In-network dentists accept negotiated fees (maximum allowable charges, or MACs) as payment in full for covered services and cannot bill members for the difference between their standard charges and the MACs. - United Concordia pays a greater percentage of dental bills for in-network visits. - In-network dentists file claims on behalf of members. **Out-of-network dentists** can bill members for the difference between their standard charges and the MACs, resulting in higher out-of-pocket costs. **Visiting any dentist:** Members may visit any licensed dentist in the U.S. and its territories. United Concordia does not have in-network dentists outside the U.S. and its territories, so international visits will incur higher out-of-pocket costs. **Finding a dentist:** Use the Find a Dentist tool at uccifedvip.com/find-a-dentist to search by name, specialty, city, county, or ZIP code. You can compare dentists side by side and get driving directions. You may also call 1-877-394-8224, Monday–Friday, 8 a.m.–8 p.m. ET. **Nominating a dentist:** Members can nominate their current out-of-network dentist to join the network by submitting the Nominate Your Dentist form at uccifedvip.com. Dentists can also call 1-877-394-8224 or download an application directly. **Specialists:** No referral is required to see a specialist. Exceptions apply for limited access areas and international care. Out-of-network specialist visits may result in higher costs. Requesting a predetermination before extensive treatment (over $300) is recommended. **Same-day cleanings:** Routine cleanings cannot be performed on the same day by the same dentist. **Family dentist flexibility:** Family members may each visit different dentists, including different in-network or out-of-network providers. **Claims:** In-network dentists file claims automatically. For out-of-network visits, members must submit claims themselves online or by mail to: Dental Claims – FEDVIP, P.O. Box 69416, Harrisburg, PA 17106-9416. **Limited access areas:** Geographic regions where OPM has identified fewer in-network dentists relative to the federal employee population. Members in limited access areas who use out-of-network dentists will have those services paid at in-network coinsurance levels. To find out if you live in a limited access area, call 1-877-394-8224. --- ## Coordination of Benefits (COB) - [Our Plans](https://www.uccifedvip.com/our-plans) If a member is covered by two or more plans, coordination of benefits ensures total payments do not exceed 100% of total expenses. United Concordia coordinates benefits with non-FEHB/non-PSHB carriers and with other group health coverage. **FEDVIP vs. spouse's coverage:** FEDVIP is primary for the member; the spouse's plan is secondary. The spouse's plan is primary for the spouse; FEDVIP is secondary. For dependent children, the plan of the parent with the earliest birth date (month and day) typically pays first. **FEDVIP and FEHB/PSHB:** By law, FEDVIP must be the second payor when the member has dental coverage through their FEHB/PSHB plan. FEHB/PSHB pays first; FEDVIP supplements. FEDVIP cannot be designated as first payor over an FEHB/PSHB dental benefit. **Enrolling in FEDVIP when already covered by FEHB/PSHB:** FEDVIP provides additional coverage beyond what most FEHB/PSHB plans offer, including extensive intermediate and major service coverage such as fillings, oral surgery, periodontal care, crowns, and dental accident coverage. **FEHB/PSHB dental HMO:** If an EOB is unavailable, the dentist may indicate the amount paid by the first payor on the claim form.
